If you have questions regarding your health issues or have a disqualifying
condition, the first thing to do is contact us. You may do this by telephone (the
preferable method) at
602-368-5633 or by sending us the requested information
on the
Contact Us page.

In your
INITIAL CONTACT with us, we will obtain details regarding your health
issue. This usually takes about 15 minutes. At the end of this discussion, we will
be able to let you know your likelihood of success in obtaining a Special Issuance
medical certificate.

You do not need travel to see us for these services. Everything can be handled by
telephone, FAX, mail, and e-mail.

RECORD REVIEW: If you wish to have us review your entire medical record, we will
let you know our opinion of whether or not you could expect to have the FAA issue
you a medical certificate. This requires copies of your complete, unedited medical
records. Once these are received, you can expect a response from us within 3 to 5
working days. This service does not include preparation and submission of your file
to the FAA.

BASIC SERVICE: If after the initial contact with Special Issuance, if you choose our
services, you will receive  agreements, release of medical information forms,
authorization for us to be your agent, and an introductory packet. Once the initial
paperwork is completed and returned to us, we will send you and/or your doctor a
letter outlining the specific documentation required by the FAA to consider your
application. Your medical records and application documents will be reviewed by
our Senior AME for completeness. We will then summarize your information and
submit your application packet to the FAA for you. As the process continues, we will
keep you informed of the progress of your application and be with you all the way.

ADVANCED SERVICE: Includes all of the elements of the Basic Service plus
individual follow-up calls to all of the health-care providers you identify as being
part of your care to insure they understand specifically what the FAA will want to
review for your case. This involves written correspondence as well as telephone
contact with these providers.

A rejection letter from the FAA does not have to mean the end of your medical
certificate and possibly your aviation career.  If you have been disqualified for a
medical certificate, you have two options: a) prove that you do not have the medical
condition with which you've been diagnosed; b) apply for Special Issuance medical
certificate.

The
Special Issuance services of  My Flight Surgeon will expedite FAA approval.
With your permission, our service retrieves, reviews, organizes and submits pilot
records to the FAA increasing your chances of a more timely successful approval of
your medical certificate.
