Special Issuance is a service of My Flight Surgeon. We at Special
Issuance services are dedicated to Pilots and Controllers with medical
Special Issuance needs. We want to see you successfully obtain and
keep your medical certificate while maintaining aviation safety.
My Flight Surgeon is an exclusive medical practice devoted to the care of
Aviators. We feel you are a unique group of people who have exceptional
requirements for your health care needs. Our practice is designed to
meet the requirements of Professional Pilots. The care of pilots is our
only specialty.
In addition to providing the medical certification process with Class 1, 2,
and 3 FAA medical examinations, we also provide comprehensive health-
care services for aviation personnel. As an integral part of those services
is the Special Issuance program where we aid you, the pilot or controller,
in evaluating medical problems that are impeding your medical
certification. Our services at Special Issuance then assist you in
navigating the certification maze.
Walter A. Forred, M.D. is the founder and CEO of My Flight Surgeon, PC
in Phoenix, Arizona. He is an experienced Senior Aviation Medical
Examiner who has been providing health care to pilots and controllers
since 1970 when he became an Army Flight Surgeon. He was the Flight
Surgeon for the 16th Aviation Battalion in Stuttgart, Germany before being
reassigned to the 101st Airborne Division at Fort Campbell, Kentucky as
the Division Surgeon. He went on to receive his residency training in
Family Practice at Fort Ord, California. After his military service, Dr. Forred
was designated a Senior Aviation Medical Examiner and has remained
so for over 30 years. He has extensive practice and administrative
experience within the private, public and academic sectors of medicine.

- Dr. Forred has had an enthusiasm for aviation
since college and holds a commercial pilot license
with instrument rating. His flying passion has been
for "tail-dragger" aircraft. As an Senior AME he is
dedicated to providing personal, high quality health
care to the aviation personnel he serves. His goal
is to keep pilots safely in the air and acting as their
advocate.
